If you are completing a certificate for the seminar series, you will need to upload your completed worksheet. When you upload your completed worksheet, please be sure to select the seminar that you have completed as well as include your email address in the upload form. 

Please note that if you are only completing a certificate of attendance, you are not required to upload your completed worksheets.
